Lodging: This Certificate entitles the recipient to five days and four nights of accommodations in a standard hotel room at one of the locations described, subject to availability and certain restrictions listed. Accommodations are for two adults 21 years of age or older and up to two children under 18. Due to the nature of this program, locations may be added and deleted at any time. No pets. This Certificate is non-transferable and may only be used by the original recipient. Destin and Myrtle Beach destinations are not available during June, July, and August. Activation/Reservations: You must activate your certificate within 30 days of the issue date. You have one year from the activation date to complete travel. Reservation requests may be made at the same time as, or after, activation. Please indicate two choices of dates and destinations in order of preference. All reservation requests require 60 days advance notice and are subject to availability. Holidays or date requests given under 60 days advance notice are $25 extra per night per person. A Reservation Confirmation Letter will be mailed to you within 72 hours of your request being confirmed. Please allow for mail time. Do not travel before receiving written confirmation. YOUR CHECK IN VOUCHER AND IDENTIFICATION WILL BE REQUIRED UPON CHECK-IN. Deposits and Fees: An $89 p.n.p.p. processing fee must be made payable to Executive Tour & Travel Services, Inc. (“ETTSI”) with your Reservation Request (rates based on double occupancy.) Additional travelers and longer stays may be available are available and may be added when confirming your reservation. Upgrades to all-inclusive resorts are also available. Incidental Charges: This Certificate is for hotel accommodations only and does not include transportation, airfare, room taxes ($3-$17 per night), meals and any other miscellaneous expenses. Upon check-in, hotels will require an imprint of a major credit card to cover such costs. Cancellations: Written notice of cancellation or changes to an active reservation must be received at least 8 days prior to your arrival date. Failure to cancel or simply not showing will result in forfeiture of your certificate and processing fee. Any changes or cancellations in reservations must be in writing and will be assessed a fee. Usage Limitations: You may not use this Certificate in conjunction with any other promotional program by ETTSI unless authorized in writing. Only one Certificate per household per year may be used. This certificate cannot be used at the same time as a friend or family member traveling to the same destination. Liability Limitations: ETTSI will arrange hotel accommodations, acting only as an agent for hotels, and will not assume any responsibility for loss, delay, accident, injury or damage to persons, property or any liability whatsoever arising or in conjunction with services provided.
